Kindergarten Requirements

Kindergarten Registration 2011-2012 School Year

Kindergarten Registration, for children in the Washington County School District who are turning 5 on or before September 1, 2011, will be held on the following dates and times:

Location:  All elementary schools in Washington County

Dates:  March 24th and March 31st, 2011

Times:  9:00 a.m.-11:00 a.m and 1:00p.m. - 3:00p.m.

To enroll you child you must provide proof of immunizations, (see Immunization Schedule link) and a birth certificate, or other reliable proof of the student's identity and age together with an affidavit, (a statement made under oath and notarized) explaining why the birth certificate cannot be provided.

An appropriate exemption form, obtainable through the Utah Department of Health, must be completed by those children who claim exemption to immunization for medical, religious, or personal reasons.

NOTE:  Children who miss the above registration dates and will turn five years of age on or before Septemeber 1, 2011, may also register August 9-10, 2011 at their perspective school. Enterprise students, please call Enterprise Elementary at 878-2236, for alternate August registration dates and times.
