NEW STUDENT REGISTRATION 2010-2011 SCHOOL YEAR

The following documents are required for registration: Proof of immunizations or waiver from the Utah Public Health Department, birth certificate or other reliable proof (affidavit) of the student’s identity, transcript or report card (secondary). Please note that some schools may require proof of residency.

ELEMENTARY SCHOOLS

• All elementary schools except Enterprise - August 4-6 (W-F) and August 9-10 (M-T) 9:30-11:30 a.m. and 1:00-3:00 p.m. No appointment necessary
• Enterprise Elementary - August 11-13 (W-F) and August 16-17 (M-T) 9:30-11:30 a.m. and 1:00-3:00 p.m. No appointment necessary
• Kindergarten children who have not previously registered and will be five (5) years of age on or before the first day of September 2010 may also register on the dates listed above. (See documents required for registration. Documentation of a health physical is strongly suggested.)
