Student Registration 2011-2012 School Year

NEW STUDENT REGISTRATION 2011-2012 SCHOOL YEAR 

The following documents are required for registration: Proof of immunizations or waiver from the Utah Public Health Department, birth certificate or other reliable proof (affidavit) of the student’s identity, transcript or report card (secondary). Please note that some schools may require proof of residency.
Kindergarten children who have not previously registered and will be five (5) years of age on or before the first day of September 2011 may also register on the dates listed below. Along with proof of immunization and a birth certificate, documentation of a health physical is strongly suggested.

ELEMENTARY SCHOOLS

All Elementary Schools - except Enterprise August 3-5 (W-F) and August 11-12 (Th-F) from 9:30-11:30 a.m. and 1:00-3:00 p.m. Enterprise Elementary August 10-12 (W-F) from 9:30-11:30 a.m. and 1:00-3:00 p.m.

SECONDARY SCHOOLS

Fees must be paid before students can pick-up their schedule. (No appointment necessary unless specified.) INTERMEDIATE SCHOOLS (Grades 6 & 7)
