Additionally, WCSD Form 122 must be completed by the employee and their supervisor. From 122 is available\u00a0on the forms page of the district website and by clicking the link below.\u00a0<\/span><\/p>\n<\/div>\n<ul>\n<li>\n<p><img decoding=\"async\" src=\"\/risk\/wp-content\/uploads\/sites\/29\/2023\/04\/company_nurse_logo.png\" alt=\"Company Nurse Injury Hotline logo\"><\/p>\n<h3>Step 1<\/h3>\n<p>Call Company Nurse<\/p>\n<div>\n<h4>1-888-375-0279<\/h4>\n<p><span>Company Nurse is an injury hotline wherein WCSD employees and registered volunteers can document injuries sustained on the job with a registered nurse. When injured personnel call Company Nurse they begin a paper trail that will protect them if their injury becomes more serious over time. ALL injuries should be reported to Company Nurse regardless of the injury&#8217;s severity.<\/span><\/p>\n<\/div>\n<\/li>\n<li>\n<p><img decoding=\"async\" src=\"\/risk\/wp-content\/uploads\/sites\/29\/2023\/04\/ihc_logo.png\" alt=\"Intermountain Healthcare Logo\"><\/p>\n<h3>Step 2<\/h3>\n<p>Go to IHC Workmed<\/p>\n<div>\n<h4>2947 East 1450 South<br \/>St. George, UT 84790<\/h4>\n<p><span>Company Nurse\u00a0<\/span><strong>may<\/strong><span>\u00a0recommend that the employee be seen by a physician. Employees who require\u00a0<\/span><span>non-emergency<\/span><span>\u00a0medical care should go to IHC Workmed. Workmed is approved by WCSD&#8217;s insurance to address worker&#8217;s compensation injuries. Employees that go to other facilities run the risk of paying for the medical costs out of pocket.<\/span><\/p>\n<\/div>\n<\/li>\n<li>\n<p><img decoding=\"async\" src=\"\/risk\/wp-content\/uploads\/sites\/29\/2023\/04\/usbia_logo.png\" alt=\"Utah School Boards Association logo\"><\/p>\n<h3>Step 3<\/h3>\n<p>Obtain Worker&#8217;s Comp Claim Number<\/p>\n<div>\n<p>Employees who seek medical care will receive their worker&#8217;s compensation claim number approximately 7 days after documenting their injury with Company Nurse. The claim number is received through regular mail from the Utah School Boards Insurance Association.
